Student-Athlete Transportation Consent Form

This form should be completed to obtain permission for Uchenna Academy coaches and students driving students to school related events.

The application form will take about 5 minutes to complete.

Uchenna Academy requires explicit permission from parents allowing their students to drive other students, and to be driven by our coaches and other students, for school-related activities including athletic events. A parent's decision to choose to not have their child to drive or be driven by our coaches and/or other students will not impact their child status on a sports team in any way. However, many times it is helpful to have coaches and senior students assist with driving, and we would appreciate you filling in this form.

I grant permission for my child to travel with a coach employed by Uchenna Academy using transportation not provided by the school. My child may:

I allow my child to travel with a coach

I DO NOT allow my child to travel with a coach

I grant permission for my child to travel from from the school to a game or tournament or from a tournament or a game to the school using transportation not provided by the school. My child may:

drive their vehicle with any staff/volunteers/students that Uchenna Academy coach approves

I do not give permission for my child to drive anyone to and from games or tournaments

My child doesn't drive to school so this question does not apply to me

I grant permission for my child to travel from from the school to a game or tournament or from a tournament or a game to the school using transportation not provided by the school. My child may:

Drive with the student (or coach) driver that Uchenna Academy coach approves

I do not give permission for my child to be driven by another student or a coach from Uchenna Academy
